Yokosuka Friendship Day - Vendor Information

• Only current home based businesses in good standing will be allowed to apply.
• Event fundraising application package deadline is Friday, September 12 at 4 p.m.
• Completed packages must be turned in to CFAY Legal.

No extensions or late submissions will be granted.

Important Notice: Any persons submitting this request must be part of the private organization’s board member list and/or accompanied by a board member and is certifying the accuracy of the documents within. Missing items from your fundraising request can potentially delay processing and result in disapproval.

Step 1 – CFAY LEGAL
Is your private organization approved to operate onboard CFAY and currently in good standing?

• YES. Please go to Step 2

• NO, we are not approved to operate. Please fill out a Private Organization Recognition Package, to establish a NEW Non-Funded Entity (NFE). You can find the package here this website or send an email to CFAYLegal@us.navy.mil.

Important: Contact the Legal office for the next Private Org Treasurer training information. Pictures of product/sales are required as a part of package submission. These completed packages are due to CFAY Legal well in advance, as your organization must be approved to operate before the Fundraising Request Form & supporting documents can be submitted.

Step 2 – CFAY LEGAL
• Please fill out Fundraising Request Form (Version G – 2023). To receive the form, please visit the CFAY Legal Office at this website or send an email to CFAYLegal@us.navy.mil.

Step 3 – PREVENTATIVE MEDICINE / ARMY VET
Will your organization be handling or preparing food?

• YES. Complete the Temporary Food Establishment Form and and obtain signature from the Army Vet & Prev Med. Alternate sources of food options (e.g. other than NEX & Commissary) must be added to this form. The Temporary Food Establishment Form can be found on the CFAY Legal website

• NO. Proceed to Step 5 or 6

Step 4 – PREVENTATIVE MEDICINE
Have all members who are preparing and/or handling food attended food handling training through USNH Yokosuka and can provide Food Handler Certificates?

• YES. Please check that each certificate does not expire prior to event date. If you have a valid certification that is not expired prior to event date, you may submit valid certificates in your package to Legal in lieu of attending class.

• NO. Please attend one of the classes listed below. Classes will be held at MWR Building 1559, Room 224 (above Starbucks). No sign up is required.

• Wednesday, September 10 • 1 p.m.

Step 5 – FIRE DEPARTMENT
Will your organization be cooking using any kind of open flame on site at this event?

• YES. Please fill out a Hot Work Permit. You will obtain a copy of the Hot Work permit at the next vendor meeting. You may also download a copy from this webpage. Fill out 2 permit sheets (4 total permits). Submit one sheet (unsigned) with Legal Packet and keep one for the Fire Deptartment, review and sign on the day of event.

• NO. Please go to Step 6.

Step 6 – CFAY LEGAL
Provide the following to CFAY Legal for processing and approval:

• Organization affirmation letter (CFAY Legal)
• Completed Fundraising Request form (CFAY Legal)
• Signed Temporary Food Establishment Form (if applicable) (Preventive Med./ Army Vet)
• Food handler certificates (if applicable) (Preventive Med.)
• Pre-filled, unsigned, Hot Work Permit (if applicable) (Fire Dept.)

Once approved...

Step 7 – COMMISSARY
All food orders must be placed with the Commissary a minimum of 8 weeks prior to event date for guaranteed delivery. Bread must be paid for at the time the order is placed. Items may not be returned after purchase. Order form may be obtained from the Vendor Meetings or requesting at the Commissary

Step 8 – MWR
There is a mandatory $350 registration fee to participate. This fee covers the cost of one (1) 10x10 canopy, one (1) 8-Foot Table, and two (2) Chairs. No power will be available for this event.

Once approved and confirmed by CFAY Legal, registration fees can be made at the MWR Tickets & Travel Office. We are located in BLDG. 1559 (adjacent to Starbucks). Please note that we accept credit/debit only.

• Registration opens Thursday, August 14
• Registration closes Tuesday, September 23

Additional logistics requests are due Tuesday, September 23 to the Special Events office or can be requested by email using the address below.
